The spec I mainly run is a 3/51/17 frost dual wield spec. It's considered a cookie cutter build for frost dps.
I've played this spec since the release of 3.2. (So about 3 months? with minor tweaks here and there.)

Typical Weekly Playtime:
It honestly depends on the week. I don't work during the raid times, so attending raids is not an issue. I play 4-5 hours on Saturdays, and an extra hour or two on all weekdays (besides raid times)
Time Zone:
PST (pacific standard time)
I've never had any issues with time zones though =P
Previous Guilds:
I'll list my main 3 (wrath)
Redux (from spirestone, a while back)
IL Seragilo (drenden alliance guild)
Anguish (another drenden alliance guild)
Previous Raiding Experience:
TBC - didn't really raid at all.
Wrath - 10 naxx full clears, 25 naxx clears, 10 ulduar hard modes, and reg. 25 ulduar, some hard modes. Almost full clear reg
10 toc normal(5/5) and heroic (3/5?)
25 toc normal(5/5) and heroic (2/5 on my druid, and pally)
10/25 voa, and onyxia.
Why are you applying to us:
I'm switching factions, and I recently saw one of your troll priests, she seemed to have decent gear, so I looked you guys up on google. Then I decided I would give it a shot to apply. Anyways, I'm mainly applying because I like the way your guild is structured.

Hmm, I actually don't have one readily available. I'd be more than happy to get one for you after our raid tomorrow night.
To give you a rough idea of my dps, it's around 5.5-6k on tank and spank fights. On fights like the beasts of northrend, it tends to go down to about 5k-5.5k
Obviously just saying that isn't enough, but I'll post one ASAP.

I don't, actually. I'm currently alliance, so being pals with horde people isn't easy for me.

Obviously I like to play WoW. I work for INTEL with an entry level internship job, and I attend the University of Arizona (majoring in biochemistry), I'm 24 and engaged. I'm a very social person, and I love getting the chance to meet new people, and mesh with new guilds.

- Why Glyph of Howling Blast instead of Glyph of Disease?
- Why are you using a fast off-hand? DW builds used the slowest off-hand possible.
- Your Sons of Hodir rep is only Honored. Small thing, yeah, but why haven't you maxed it out for the shoulder enchant?
- Most raiders try to min-max. To that end, why don't you have any professions?

Everyone asks the same questions, and I'm glad to answer them. Currently, the glyphs I'm using keep my dps pretty equal to what the disease glyph would do. I have tested it, and this best fits my play style (though it might not be others preferences, this works best for me.)
To address the fast off-hand. To be completely honest, it was either that or the one handed mace from toc 5 heroic. I do plan on replacing it, I've actually spent a lot of time trying to get a new one. It has been my #1 priority in my gear, along side the trinket I need to replace.
Hodir rep.... yeahh, that's been an issue. I'll probably do that tonight or tomorrow night.
Professions were never my top priority, but if you guys would require that I go get them, I'll go get my JC and enchanting up. (yeah, I already have the money, so skinning wouldn't be my top choice)
Thanks for the feedback, I obviously have a couple things to change when I have my 30 day lockout.

As for professions, they certainly aren't required, but most people will do what they can to squeeze every last drop out of their character. With JC for example, you'd be gaining 42 strength by upgrading 3 gems to Dragon's Eyes. I can't tell you that's worth leveling JC to 450 but in the sense that all it takes to get there is money, it doesn't seem like the barrier to getting there is very high.
Biochemistry and working at Intel? Are you on the Terminator project or what? Please make them not hate humans!

I always thought the weapon speed was inversely related to the damage per hit. But I don't know the system used for scaling melee damage so pretend I never said anything.

If there's one thing I know, it's DK DPS. Needs moar Spellpower IMO.
